I, _____________________________________________hereby agree to engage Marilyn Henrion (aka The Soho Bookie) to handle all aspects of publishing an on-demand print version of the book entitled___________________________________by___________________________________________ as stated below.
SERVICES
After an initial consultation, Marilyn Henrion will design and format my book from images and text provided by me. I will have an opportunity to make any design revisions on a printed proof before final approval. An additional fee of $50 par hour will be billed for any revisions after the first proof. Marilyn Henrion will handle all of the createspace account registration, submission, and production aspects through final proof approval and publication. After that, the account will be turned over to me for future ordering and managing (no additional work will be needed on the book).
FEES
I agree to pay _____________________________ to Marilyn Henrion for the services described above, Payment is to be made in two installments, 50% upon signing this contract, the balance when the book is published and createspace account is turned over to me.
